At HCA Florida Trinity Hospital, we know your time is valuable. That’s why we offer convenient online registration for services such as surgical procedures, mammograms and diagnostic tests and treatments.

HCA Florida Trinity Hospital strives to give patients the best experience possible by creating and carrying out patient-focused policies and procedures.

We want you to be as comfortable as possible and very satisfied with your hospital patient experience. This information will help you prepare for your admission and hospital stay.

Upon admission to the hospital, you will find the below patient guide in your room, which may be helpful to your and your visitors.

Before your hospital admission

Your doctor arranges for your admission, decides what tests and procedures you receive, prescribes your medications and diet in the hospital and decides when you are ready to be discharged. It is important that you discuss your care with your physician while you are hospitalized at HCA Florida Trinity Hospital.

What to bring and what to leave at home

Leave unnecessary valuables at home. Be prepared to pay your co-payments and deductibles, but do not bring large amounts of cash with you. We cannot be responsible for loss or breakage of personal items brought into the hospital.

Advance directives

If you have a living will, bring a copy when you report to Admitting. If you want to have one in your medical file, the Admitting Office has forms available. It is advisable for you to complete a living will, which would contain your advanced directives, and to appoint a healthcare surrogate and/or advance directive proxy since not every possible situation may be covered by your living will. This plan for future health care issues only goes into effect if you become mentally incompetent and therefore unable to communicate your wishes. Please remember that these advanced directives can be changed by you at any time until they are put into effect and are never used as long as you are able to make and communicate your own health care decisions.

Another option is a healthcare surrogate. This is a person chosen by you to make health care decisions for you when you are unable to make them yourself. You need to be sure this person completely understands your wishes since they can make decisions concerning withdrawal or withholding of life-prolonging procedures.

Your living will can aid your advance directive proxy and healthcare surrogate in determining what your wishes would be under many circumstances. Refer to the pamphlet in the back of this folder for forms and further information on these choices.

Insurance information

Bring your insurance card when you are admitted. We make a copy of it so we can bill your insurer directly for your covered benefits. Your co-payments are expected at this time. It is important that you understand what your insurance covers and where you can go for care. Many insurance plans require pre-certification before admission. To avoid reduced benefits, call your insurance company to verify your coverage and make sure your authorizations are in order. If you have any financial questions, please call our Business Office at (727) 834-4797 during weekdays from 8:30am to 5:30pm.

Private patient room features

Every patient’s comfort and privacy will be enhanced with a stay in our uniquely designed, private rooms. Each comes with a restroom, shower, 32” flat panel high definition television, Internet access, ample storage, and plenty of room for visiting family, friends and loved ones. To enhance further discretion, HCA Florida Trinity Hospital has been built with dedicated patient hallways and elevators for optimal privacy while accessing other service areas.

Discharge from the hospital

At HCA Florida Trinity Hospital, we believe planning for your recovery is essential. As part of your healthcare team, our case management staff works with your physician to ensure that proper arrangements are made before you leave our care. During your hospital stay, it may be determined that you need assistance to provide transportation home, home care, medical equipment, referral to a skilled nursing facility or an adult living facility and/or information about counseling, community services, support groups, etc.  A member of our case management staff will be happy to meet with you to discuss your individual needs and coordinate these necessary arrangements. Our case management team can be reached by phone at (727) 834-4028.

When your physician determines that you are ready to leave the hospital, your nurse will review the discharge instructions with you regarding your care once you leave the hospital. Please do not hesitate to ask questions. When you are ready to leave, our nursing personnel or a volunteer will escort you to the main lobby and wait with you while your driver brings the vehicle to the front entrance to meet you.
